Is John really her husband? Or is he just her doctor, and she imagined the whole thing? When he is in the room with he at night could he not have been making observations? He always has other patients that he have to be tend to over night so couldn't she be just a patient too? If she truly was in a mental institution wouldn't this make sense?

I think you make a good point there. I had that same thought throughout the story. He didn't really treat her like he loved her anyways. He just told her not to write and not to do anything that might make her tired. That was as much care as I found he had for her. Except when he was trying to get into th room with the axe; he was desperate to save her. Perhaps though, our perception as readers is a little skewed because the story is being told by a crazy woman. We may never know.
